About Nationwide Health Plans
Based in Columbus, Ohio, Nationwide Insurance
is rated A+ by A.M. Best with assets exceeding $100 billion. The
California division is based in Sacramento Nationwide contracts with the
CCN provider network of physicians which allow you access to in-network
physicians in 48 states. Nationwide is an association health plan and
enrollment requires membership in the County Farm Bureau Federation
(annual dues range from $40 to $75 per family). This is the first and
oldest association health plan in California. Nationwide has several PPO
plans available with a wide range of premium and coverage options.
Nationwide PPO Plans
About the Nationwide PPO Choice 15 Plan
This plan offers you a choice of 3 deductible options: $500, $1,000 or
$2,500. Office visits and preventative health care are $15 without having
to meet your selected deductible first. For other covered services, you
pay a 10% copayment for covered services, up to an annual out of pocket
maximum of $2,500 (not including your selected deductible). All Nationwide
health plans include prescription benefits and $5,000 of accidental death
coverage.
About the Nationwide PPO Choice 45 Plan
This plan offers you a choice of 2 deductible options: $750 or $1,000.
Office visits and preventative health care are $45 without having to meet
your selected deductible first. For other covered services, you pay a 25%
copayment for covered services, up to an annual out of pocket maximum of
$4,000 (not including your selected deductible). All Nationwide health
plans include prescription benefits and $5,000 of accidental death
coverage.
About the Nationwide PPO Choice Classic Plan
This plan offers you a choice of 2 deductible options: $2,500 or $5,000.
Office visits and preventative health care are $40 without having to meet
your selected deductible first. For other covered services, you pay a 10%
copayment for covered services, up to an annual out of pocket maximum of
$2,500 (not including your selected deductible). All Nationwide health
plans include prescription benefits and $5,000 of accidental death
coverage.
About the Nationwide PPO Choice Select Plan
This plan offers you a choice of 2 co-pay options: $25 or $40. The annual
deductible is a low $250. Office visits and preventative health care are
$25 or $40 without having to meet your selected deductible first. For
other covered services, you pay a 40% copayment for covered services, up
to an annual out of pocket maximum of $5,000 (not including your selected
deductible). All Nationwide health plans include prescription benefits and
$5,000 of accidental death coverage.
About the Nationwide PPO Choice Advantage Plan
This plan offers you a choice of 2 deductible options: $1,500 or $2,500.
Preventative health care is provided at a 30% co-insurance without having
to meet your selected deductible first. For other covered services, you
pay a 30% copayment for covered services, up to an annual out of pocket
maximum of $3,500 (not including your selected deductible). All Nationwide
health plans include prescription benefits and $5,000 of accidental death
coverage.
Added Benefits
Membership in the California Farm Bureau
provides you with benefits such as long distance phone discounts, low
interest member credit card, 50% off on hotels and resorts, and many other
discount guarantees from associated vendors and retailers.
MedPerks is an optional program that
offers discount healthcare products and services to CalFarm members.
Benefits include vision care, hearing, chiropractic, dental, home medical
supplies, vitamins and supplements, pharmacies, a 24 hour medical help
desk, physician referral service and evacuations and repatriation
services.
Out-of-State Benefits
Nationwide contracts with the nationwide CCN directory. While traveling
outside the State of California, you can access any physician within this
network and obtain health care at "in-network" discounted rates. The
MedPerks physician referral service will assist you in finding these
nationwide participating physicians.
Waiting Periods
For PPO plans, there is a specific six-month waiting period for coverage
of any condition, disease or ailment for which medical advice or treatment
was recommended or received within six months preceding the effective date
of coverage. If you apply for coverage within 63 days of terminating your
membership with another "creditable" health care plan, then you can use
your prior coverage for credit toward the six-month waiting period.
Enrollment Process
Enrollment requires the completion of a somewhat lengthy and detailed
health questionnaire. You may apply online, download an application, or
request a detailed plan brochure and application. A refundable deposit in
the amount of the first month's premium must be included along with the
completed application.
